Californias Department of Health Care Services Agency has renewed its 1115 Medicaid wavier and will distribute funds for Providing Access and Transforming Health (PATH) Program.

PCG has been identified as the Third-Party Administrator for this effort.

As a part of the effort to advance healthcare outcomes in a collaborative and comprehensive manner, PCG will work with Managed Care Plans, Enhanced Care Management and Community Supports contractors to support State-wide Collaboratives that will support system wide integration and improvement efforts.

Responsibilities Essential Duties and Responsibilities: Serves as program manager and subject matter expert over a justice-involved grant program with a strong understanding of justice-involved individuals, re-entry from prison and jail, and the intersection of justice and behavioral health in the community.

Understands challenges associated with justice-involved individuals accessing Medicaid services and Medicaid eligibility upon re-entry into the community.

Manages justice-involved grant program operations, including grant application reviews, grant application scoring, and creating recommendations for program funding.

Manages justice-involved grant program budget.

Understands grant management processes and best practices.

Analyzes and interprets federal and state law and regulation.

Independently manages complex projects, staff and tasks with limited supervision while meeting and exceeding performance expectations.

Facilitates client discussions regarding grant program design and reporting.

Identifies gaps and shortcomings and determine necessary workarounds.

Plans and monitors ongoing project tasks.

Delegates tasks and responsibilities to team members.

Responsible for supervision of Grant Support Lead.

Required Skills Strong quantitative skills and able to analyze and interpret financial data.

Ability to research, analyze and interpret policy, legal briefs and regulations.

Knowledge of Microsoft Suite including, Excel, Word and SharePoint.

Ability to manage time effectively in a fast-paced environment.

Proven analytical and problem-solving abilities.

Able to effectively manage a team of grant staff supporting the grant operations.

Qualifications Desired Experience Experience with grant management, application processing, application reviews, etc.

Experience with managing complex business processes and workflows Experience leading teams Experience in health & human services or community-based services fields preferred.
