– Social Services Case Manager Position Summary: This part-time position will provide case management for the clients at the maternity home, cover residential specialist shifts at the maternity home and assist in other programs during times of low census at the Light House in Kansas City, MO.

This maternity home position is a 20-hour per week and a flexible 1 day evening per week to meet client needs (no later than 8 pm.) Traditional case management position in this position is with a maximum of caseloads of 6 clients.

This is a part-time position but open to full-time as well for this position.

Case Management: Provide assessment, intake, orientation, and welcome materials for new clients Document appropriately, including data entry into required databases Case management of client treatment plans and management of client binders Coordinate weekly client review Create a weekly schedule that plans for events, transportation, and client needs Confirm all passes and visits Check the mailbox twice weekly and deliver mail to clients Ensure client grievances follow the agency procedure Be on call 24/7 for urgent/emergency situations related to clients Coordinate and communicate with the Maternity Home manager to ensure continuity of care Residential Advocacy: Perform all Residential Specialist responsibilities and fill open shifts Qualifications Bachelor’s degree in social work or related field preferred Experience working with teenagers, single parents, homeless and care of newborns.

General knowledge of running a household and ability to teach/instruct others relating to necessary life skills.

Must demonstrate a strong faith in Jesus Christ.

Must be 21 years of age, pass a Child Abuse/Neglect check and fingerprint verification, possess a valid driver’s license with an acceptable driving record and current car insurance.

Must be able to lift up to 30 pounds and care for infants and toddlers without assistance.

Flexible schedule with some evening availability as needed
