Monogram Health supports patients suffering from Chronic Kidney Disease and End Stage Renal Disease by forming deep rooted relationships and preparing them both emotionally and physically for the challenges of managing kidney disease.

Monogram Health uses next generation artificial intelligence algorithms to predict necessary and timely care to promote the delay of kidney disease progression, seamless transitions to dialysis and/or pre-emptive kidney transplant.

We then utilize in-home visits to build meaningful relationships between patients and their care team that drive positive behavior change and optimize our patients’ health outcomes.

Position: Social Worker Coordinator The Social Work Coordinator (SWC) role is a key role within the clinical operations of Monogram Health.

This role will assist Social Workers with scheduling specialty clinic, physician appointments, and set up transportation blankets after in person initial enrollments, follow up, and post hospital visits.

The SWC will be responsible for contacting health plans as needed to inquire about coverage and/or to change PCP or refer to specialty programs as needed.

In addition, they will contact members to determine if they followed through with scheduled appointments.

Roles and Responsibilities Research market community-based resources for housing, food, transportation, utility, medication, or caregiving programs and provide that information to the Social Workers.

Conduct three-way calls with member when needed to apply for community-based programs or health plans to inquire about coverage.

Prepare, review, and analyze Medicaid conversion progress reports to identify areas requiring attention and improvement, acting quickly to resolve known issues Research and maintain the central resource library in Shared Drive.

Assist with referrals for other community financial resources.

Assist with ALTCS applications as needed Update lab information and look up information in HIE for GFR and A1Cs.

Request 2728 document when contact dialysis clinics and upload to Salesforce/Health Cloud.

Complete timely documentation in Salesforce/Health Cloud and close out any outstanding SDoH.

Participate in weekly case rounds to identify care team support needs.

Provide support in special projects as needed.

Support Monogram as necessary with any tasks required to deliver excellent personalized kidney care management and perform all other duties as assigned Work closely with SW CPM to ensure follow up Social Work visits are completed.

Available to receive special projects from SW CPM and VP of SDoH Work hours that align with market needs.

For example, if SWC is assigned to CT zone, he/she will be available to assist during normal business hours for that time zone.
