Join the AccentCare team and apply for this Patient Care Manager opportunity today Responsibilities: Patient Care Manager ResponsibilitiesResponsible for communication & coordination of care needs reported by case managers (driveway calls).

Ensure schedules are updated, continuity of care for patient episode is present, and that quality care is delivered by the clinical care team.Facilitate Case Conference and be responsible for follow-up action related to case conference, stand up, and stand down meetings.Coach and mentorship of clinicians and CSS role – including monthly 1:1 session to address development needs and hold accountable to role specific Key Performance Indicators (KPIs).

Support development of new hires and tenured employees.

Responsible for voluntary attrition of assigned team, acting as the main point of contact for support and working develop solutions to potential personnel concerns.Responsible for daily completion of the following tasks: daily management of KPIs, field staff support, order approval, follow-up on pending admissions, prep/perform utilization management review, workflow, edit/lock TIF/Discharge OASIS, escalation/complaint calls, supervision to ensure implementation of agency policies & procedures, and field visits as required.

Qualifications: Patient Care Manager QualificationsRegistered nurse with current licensure to practice nursing in the practicing state.Previous experience in home care setting with two years management or supervisory experience, preferred.Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
