Mental Health Rehabilitation Worker

Maple Star Nevada offers in-home services, utilizing a client-centered and strength-based approach. Maple Star’s Psychosocial Rehabilitation Workers teach skills through modeling, role-playing, and educating clients on a variety of skills and work with clients to implement these skills to encourage independence, behavior management, social competency and self-sufficiency. Maple Star Nevada is a subsidiary of Pathways.
Reporting directly to the Supervisor, Coordinators are responsible for supporting agency programs in accordance with HIPAA, Quality Assurance, and Licensure regulations and coordinating client services. Maintenance and good working relationships with other agencies and organizations is essential. This is a full-time position.
Duties and Responsibilities:

  • Facilitate day treatment program and apply curriculum during program sessions, implement behavior management, social, skills, and communication skills interventions with clients in program.
  • Complete documentation for services provided during day treatment program.
  • Client file maintenance including but not limited to: conducting chart audits, filing, and client eligibility.
  • Coordinate client transportation to and from agency program.
  • Coordinate and follow up with client referrals and appointments.
  • Maintain current case notes and documents related to client caseload and in a timely manner.
  • Complete service requests and submit to insurance for approval. Complete and submit reports as required for agency information and evaluation system.
  • Facilitate employee trainings, including but not limited to: new hire trainings/orientation, ongoing agency trainings, track employee annual trainings/current documentation.
  • Participate in team meetings
  • Support agency programming and billing processes.
  • Maintain positive working relationships both internally and externally.
  • Oversee psychosocial rehabilitation services provided to clients and provide guidance and training as needed.
  • Complete in-home visits and communicate with client treatment teams on client progress and challenges.
  • Additional duties as assigned

Minimum Qualifications:

  • Must have a demonstrated ability to organize time and work so tasks are completed in a timely manner.
  • Must have a Bachelor’s degree in Psychology or related field
  • Must demonstrate good organizational skills, with an ability to work with a variety of types of people.
  • Both written and oral communications must be at a high level.
  • Must be comfortable with multi-tasking
  • Must possess a valid driver’s license, be able to operate a motor vehicle and must be willing to work irregular hours.
